Charged for replacement device not received

Can someone please help me? I have been reading this forum and seems like it's a common thing for Att to charge its customers for phones that have been returned but they claim they have not received. I was just charged $649.99 for a phone I returned back in October and after hours of being transferred around I am left with no solution and I can't afford to pay for a phone I don't have. Can someone offer any advise?
