I am shocked and ANNOYED with this Company

Since June 24, 2014, I have tried countless times to confirm the delivery of a Samsung Galaxy smartphone that I returned to AT&T. I promptly returned the phone upon receipt, and every single time I call the customer care line, I am told, "It shows that we have received the phone" and then 30 minutes into the call I hear, "I am sorry we cannot confirm receipt of the new phone.".  I have received case numbers through text messages assuring me that this issue will be resolved and to this date (8/12/2014), I have received no response.  I paid for the taxes at the time of purchase and AT&T continues to assess the installment fee (of $27.09) to my account on a monthly basis though the phone has not been activitated and it was never used. Once I received the phone, I literally returned it to AT&T within a week later (as I was instructed to do by a customer care rep.).  I have proof that this phone was returned. I just want my money back and my account to reflect accurate charges again. Please someone help.
