ATT customer service re incorrect billing is atrocious

I bought an iphone 6s at Costco and returned the next day since it would not interface with our back end legacy software system. It took me 3 months to have the cost of the phone removed from my bill. Several AT&T representatives including two supervisors (one is Ms. Yolanda Voss at a Florida AT&T help center) told me that the $89.15 overdue amount also would be removed. It never did. I have the chat session on Jan 26, 2016 with a Juan and he repeatedly told me that it would be removed. It has not. This is such an atrocius customer service. Would someone please help?
