Wrong installment plan and did not add insurance

I just noticed that I am on a 24month installment plan. I specifically told the sales rep at the point of purchase that I wanted the 18 month installment plan. Also, I wanted wireless insurance and asked for it and it looks like it was never added. These are 2 major problems. I've been with At&t a long time but if these problems aren't solved I will cancel service. The in store sales rep obviously made a lot of errors.
