Unresolved issues

How do I contact the corporate headquaters for AT&T?  I purchased a phone on May 24th and returned it on May 25th. I never should have been billed but received a bill for $70.04. I called in and after 3 calls I finally got that issue resolved. Since then I have been billed for a $5 late fee for the bill that I should have never received in the beginning. Now I am receiving a collections notice in regards to the $5 because no one can seem to remove it from my account. This is just utterly ridiculous. I shouldn't have had to call 5 or 6 times and then try to chat with an agent to get this matter resolved. The worst part about it is the problem still exists after all these efforts. Joyce (a chat agent) was an amazing rep and she listened and I can tell she really cared. I really appreciate her. However, I want this matter taken care of.
