I am being charged for wireless after changing plans. Existing plans not canceled as stated by phone

I had wireless plan with AT&T. They suggested I change plans for cheaper services. I changed plans twice. They were suppose to cancel old plans. The AT&T representative did not cancel old plans and my 30 days period to cancel my service had elapsed. I was turned over to a collection agency for non payment. This bill was also turned over to my credit report causing my credit to be lowered. I also had AT&T universe service at the same time and the bills were all paid on time. Can you explain how this could happen and what needs to be done to get it off my credit report?
