defective warrenty replacment problems

My Samsung galaxy stopped working it wasn't even a year old I called and had them ship me a replacement phone. I was very happy because they waived the fee and did priority shipping at no cost to me. Before I received that phone my original phone started working again. Instead of transfering all my stuff I decided to keep my original and send the replacement they sent me back. What would you know the day I sent that phone back my original phone broke again. So I rush to the att store had the worst customer service imaginable all I wanted to do was switch to an old I phone I had while I waited for att to receive my replacment phone before they shipped another one. They ended up crediting my account to make up for the bad service. They received the replacment phone and shipped another one out the lady would not waive the fee like the man did the first time. That's fine. So I ship out my broken phone. October 24th I received a text saying thank you your package is scanned and we will send you an email when it reaches our warranty department. Today I check my bill I'm being charged 500$ for a phone they "didn't receive" I called immediately and was told that warranty was the one that put the charge on an to contact them I contact them they said it was a billing problem I call billing back had a gentalmen checking in on it and got disconnected call back and a female I spoke to said she could see that they did receive it and was checking to see if it was damaged but didn't see anything. I need this issus resolved my phone wasn't damaged sending it in and why on my bill is it saying not received when I got a text saying it was scanned and in transit?? I have never had so many issues with ATT as I am the past few months.
